plumbing/transport: allow AdvertisedReferences being called multiple times. (#165)
* AdvertisedReferences is now part of transport.Session.
* It is allowed to be called more than once.
* It is allowed to be called before and after FetchPack/SendPack.
* Implementations cache its result.
